Medical Information

Information about students medical conditions are recorded in the School Administration System to ensure we are able offer the relevant care while they are at school. It is important to inform the school of any changes to your child's medical information and or needs as soon as possible so we can maintain accurate and up-to-date records.

Parents and carers are required to complete a Medical Information and Consent Form every year their child attends our school. The information you include on these forms will allow us to maintain our records and put in place any plans to assist in managing their medical conditions.

Medical Information and Consent Form

Upon enrolment, and each calendar year parents or carers are asked to complete a Medical Information and Consent Form for their child/ren. Information provided on this form will be maintained in the school administration system to enable us to care for your child. This form can be completed any time throughout the year should your child's medical information require updating.

Known Medical Condition Response Plan

If your child has a medical condition or medical requirements that require staff to be aware of or provide personalised or additional care for, you may be asked to complete, in addition, a Known Medical Condition Response Plan. Medical Conditions include, but are not limited to Asthma, Anaphylaxis, Diabetes or Epilepsy. Please speak with front office staff if you are unsure or need further advice about this form.

Medication Authorisation and Administration Record

A Medication Authorisation and Administration Record must be completed to accompany every medication which is held onsite for administration by staff. This form must be updated annually for long term medication requirements and must be places on student files at the end of the school year or nominated period.

Asthma, Anaphylaxis, Diabetes and Epilepsy

Parents or cares of students diagnosed with Asthma, Anaphylaxis, Diabetes and Epilepsy must also provide a personalised condition specific action plan completed and signed by their doctor.
