Media Events - Permission
From time to time students and parents at Chapman Primary will be involved in activities which may be of interest to the Media, may be useful in promoting the school through Information Brochures or through the departmental and/or our school's website. To acknowledge and celebrate student achievement and children's work, including writing, artwork and photographs of school/class activities may also be used in the Communicator, our newsletter. To assist students in observing and understanding strategies they use to achieve specific outcomes, staff at the school may also wish to video students engaging in learning tasks.
Permission to, or not to be involved in these activities, photography and/or video recordings along with permission to, or not to have children's work and/or names published is given when first enrolling at Chapman Primary School on the online enrolment form. It is important that parents or carers notify the school of any required changes to the original selections to ensure this information is maintained on the School Administration System.

Medical
The school staff have a duty of care to students to provide first aid assistance when required, it is the responsibility of the parent to notify the school of any medical issues and/or medications that their child is taking.
All students must return:
- Medical Information and Consent Form annually
Students with a known medical condition which DOES NOT require medication must return:
- Medical Information and Consent Form AND
- Known Medical Condition Response Plan
Students with a known medical condition not listed below who REQUIRE MEDICATION to be administered at school must return:
- Medical Information and Consent Form,
- Known Medical Condition Response Plan AND
- Medication Authorisation and Administration Record
Students with Asthma, Anaphylaxis, Diabetes and/or Epilepsy must return:
- Medical Information and Consent Form,
- Known Medical Condition Response Plan,
- Medication Authorisation and Administration Record AND
- Management plan from from:
- Asthma: National Asthma Organisation Website
- Anaphylaxis or Allergy: Australian Society of Clinical Immunology and Allergy Website
- Diabetes: Diabetes Management Plan downloaded from Diabetes Victoria Website (click on “How we help” and “Schools and early childhood settings”)
- Epilepsy: Epilepsy Action Australia Website
- Hard copies of all plans can be collected from the school front office
Students who require short term (less than 10 days) or over the counter medication such as pain relief, seasonal allergies or cold and flu medications must return:
- Medication Authorisation and Administration Record
- ALL MEDICATIONS must be in original packaging with the child's name and dose instructions clearly labelled.
- ALL MEDICATIONS must be stored securely at the front office and be administered by first aid trained staff.
Students attending Camp, sporting events, Swimming and Water Part Aquatic Activities and some special excursion opportunities may be required to complete additional medical forms for each activity.
Hard copies of all medical forms are available from the front office.
